REFERRAL PROCESS AND SUPPORT
We will manage your patient referrals for PCP’s, including, but not limited to, home health, hospice, and post-acute care. With our EHR System or the physician’s local office. Our support process for referrals avoids delays, or missing referrals, with a proven history of satisfied patients.
We have a physician lead team, overseeing the continuity of care and patient satisfaction.
- With our network of specialists for each plan, we will ensure that the continuity of care is kept for the patient thus allowing the primary care physician to manage their patient and not the specialist.
- We verify and schedule the patient with the specialist who can service them.
- We facilitate the process for all prior authorizations needed for the primary physician’s patients.
- We ensure that all specialists notes have been received to the back to the primary care physician prior to scheduling follow-ups for all referrals.
We receive and respond to referrals via notifications from electronic and non-electronic sources. You will never have to deal with missed or delayed referrals or manual re-entry of referral data resulting in 10 to 15 minutes of savings per patient.
